How to Apply for Health Plus

Starting your Health Plus coverage is simple.

  • Choose the plan that's right for you - PRIORITY or OPTIMUM. If you're undecided, that's okay. View our coverage page to review the plans or submit an application and an advisor will get in touch to help you figure out what's right for you. 
  • Complete and submit your application online or print an application and submit by mail or email. 
  • After you apply: We’ll review your Application and be in touch by phone or email  within 3 business days to confirm your coverage.  Your insurance will take effect the first day of the next month (unless you request a later date).  You'll receive your Health Plus™ Welcome Kit by email and your ID card will be mailed to you in about 3 weeks. 

What you need to apply ... 

  • Name, date of birth, doctor and dentist contact details for you and any dependents to be covered.
  • Details of prescription medications you or a dependent take, if applicable, including name of the drug(s), DIN #, and cost. You can find this info on your pharmacy receipt but if you're not sure, we can help. 
  • Your banking info for monthly billing and direct claims reimbursement. Don’t worry we do not process any payments and you will not be charged until an advisor has confirmed your coverage directly with you.
